After getting proper study time in, I'm lowering my difficulty rating on this test. I passed it this time with a 734, and am sure I got nearly all of the material correct. I don't think there is a very high score ceiling on this exam...Microsoft expects you to do exceptionally well to pass.
My first failed exam

I had doubts going into this exam - I scheduled it with too little time to prepare and didn't think to reschedule it. After sitting it, I thought I was doing well enough to pass. I crossed my fingers after the last case study, and failed with a 625 under 700. I'm rescheduling it for 3/31, and studying my butt off! It's the last exam I need for my 2000 MCSE, and I am taking the 70-292/70-296 back to back on 3/28!
Passed this time!

Finally passed (failed last time) after much intensive studying, re-read course notes (note the course won't fully prepare you for this exam), used Transcender questions, read Sybex 70-219 2nd edition book. Recommend you know DNS - delegated domains, integration with other DNS servers, versions, when to use AD integrated or not. Sites - know when to use sites, how to optimise replication, what site links are available. Forests/domains - know when to create multiple trees, when to trust other forests, how domains trust each other, when to consolidate domains, how to uprgade from master/resource domains, upgrade sequences for domains. Security - know when to use domain local, global and universal groups and how they should be used with each other.
Make *sure* you read the scenario. You'll always hear that but it really is crucial, a single sentence can completely sway how you will decide to implement solutions. Make sure you refer back to the scenario - all the information
Quite difficult

I found that the amount of reading and note taking got to me. I passed with 890 from memory, but it took me a fair while to pass it. My job role requires me to gather such data which did make it easier for me to find the bits that did matter, but it was allot of going back and forward, and I used three 2 sided boards.

Much too much reading required

This is a toughie; as has been said before, the sooner you can write this exam after writing the four core, the easier I think it would be.
There is a lot of reading involved, but the actual amount of useful information is probably 30% - 40%. I passed with a 728, had no idea if I had passed until the test was over. I think I will be very careful before doing these tests with Case Studies.

The exam was very dificult and in the case for people like me whos native language is spanish the study cases were so time consuming to understand. I passed but it took me more than an hour to read all the sudy cases.

BTW I did pass, second attemp though, first score 560 this time 712. Requires strong english comprehension skills, MS will try to trick you with lots of unrelated data. 1 case study was very easy, 2 moderate, the lastone a nightmare. 39 questions total.

Took the exam this morning, seems like the more complex the case studies became, the time allotted to complete the case study diminished.
